New Patients

We are only seeing Patients who are PAID members of Stafford Family Care.

If you’d like to Establish Care with us, the cost will be as follows:

New Patients: Under 1 year of Age = $85.00 ( $20.00 for Private Membership, $65.00 for first visit, Non-Refundable )

New Patients: 1 - 17 years of Age = $130.00 ( $20.00 for Private Membership, $110.00 for first visit, Non-Refundable )

New Patients: 18 years of Age and Older = $140.00 ( $30.00 for Private Membership, $110.00 for first visit, Non-Refundable )

Terms and Conditions:

• These payments will be collected at the time a New Patient appointment is made. 

• The Private Membership will be an annual cost, renewing in January of every year.

• The membership can be canceled by you, or by us, at any time, but it MUST be done in writing.

• The membership payment will not be refunded.

Initial Appointment to Establish Care can be rescheduled if needed, but it must be completed within 60 days of Deposit or funds will be non-applicable.
